Job Title: Travel Registered Nurse (RN) – Case Management

Facility Overview

Portsmouth Regional Hospital is a full-service medical facility located in Portsmouth, NH. The Case Management department plays a vital role in coordinating patient care, discharge planning, and ensuring continuity of services across the hospital. The team includes 3 Social Workers and 10 RN Case Managers, supported by a manager and director.

Unit Details

• Unit Name: Case Management
• Floor: Case Management
• Building: Main Hospital
• Staffing:
  • 3 Social Workers
  • 10 RN Case Managers
  • 1 Manager and 1 Director
• Patient Load: 25–40 cases per RN
• Documentation Systems: Expanse & CarePort
• Dress Code: Business Casual
• Floating: May float to other assignments as needed
• Weekends: No weekend shifts required

Responsibilities

• Perform inpatient case management including discharge planning, guardianship, and surrogacy coordination
• Collaborate with families and interdisciplinary teams to plan long-term care placement
• Utilize Expanse and CarePort for documentation and care coordination
• Support revenue integrity and utilization management

Requirements

• Experience:
  • Minimum 2 years of acute hospital RN case management experience
  • Critical care experience preferred
• Licensure:
  • Active Compact RN License (required at submission)
• Education:
  • BSW or higher
• Certifications:
  • BLS (American Heart Association or Red Cross)
